There can be no stigma associated with the use of extensions by any application, project, or standard - regardless of the institution or jurisdiction that uses or defines the extensions. The use of extensions is what allows the FHIR specification to retain a core level of simplicity for everyone.

Modifier extensions allow for extensions that cannot be safely ignored to be clearly distinguished from the vast majority of extensions which can be safely ignored. Business identifiers assigned to this procedure by the performer or other systems which remain constant as the resource is updated and is propagated from server to server.

This is a business identifier, not a resource identifier (see discussion). It is best practice for the identifier to only appear on a single resource instance, however business practices may occasionally dictate that multiple resource instances with the same identifier can exist - possibly even with different resource types. For example, multiple Patient and Person resource instances might share the same social insurance number.

Allows identification of the procedure as it is known by various participating systems and in a way that remains consistent across servers.

There are many sets of identifiers. To perform matching of two identifiers, we need to know what set we're dealing with. The system identifies a particular set of unique identifiers.

For example, it would not be legitimate to send the identifier for a drug prescription if the type were Reference(Observation|DiagnosticReport). One of the use-cases for Reference.identifier is the situation where no FHIR representation exists (where the type is Reference (Any).

• apenas a(s) ocupação(ões) do(s) profisssional(is) que realizou(aram) o procedimento, seu(s) Estabelecimento(s) de Saúde inscrito(s) no CNES e o Estabelecimento de Saúde Terceiro (quando aplicável) - OPÇÃO ALTERNATIVA quando o profissional não está inscrito no respectivo CNES ou o modelo de origem não disponha dessa informação;
• apenas a(s) ocupação(ões) do(s) profisssional(is) que realizou(aram) o procedimento, a(s) organização(ões) identificada(s) pelo CNPJ (pessoa jurídica) ou CPF (profissional liberal) onde o procedimento foi realizado e o Estabelecimento de Saúde Terceiro (quando aplicável) - ÚLTIMA OPÇÃO, utilizar apenas quando o estabelecimento de saúde não possui inscrição no CNES ou o modelo de origem não disponha dessa informação.
